Registration Form: STEM PD Camps for Teachers

  • To expedite your registration, please have the following items ready: 1. Specific STEM PD Camp Title and Session Dates (two-day workshops) 2. School District contact information 3. School District Purchase Order (P.O.)
  • Substitutions are allowed at any time - just let us know. If you are cancelling your registration up to 30 days prior to the date of the PD Camp Session, you are entitled to a full refund. If you cancel 20 to 29 days prior to the session, you are entitled to a partial refund in the amount of $500. There are no refunds given less than 20 days prior to the session. Please note: if you do not cancel and do not attend the STEM PD Camp Session there will be no refund given.
  • By submitting this form with necessary information you are acknowledging your acceptance of the following release: I authorize Vista Teach Instructional Services to have, take, and use without payment, any photographs and/or video of my camp participation, as may be needed for public relations purposes, as well as website and related postings for marketing/advertising purposes.

  • $795.00 per participant/per session For a group of three or more participants (from the same school) registering for the same or different PD sessions, we offer a group rate of $725.00 each (reflected in pricing below).
